Abstract
Introduction: Ray resection is indicated in ischemic necrosis involving metacarpal, dysfunction of proximal inter-phalanx joint, and amputations at the level of the proximal phalanx. The resection should allow for wound closure without tension or excess integument while preventing contracture. To the best of our knowledge, there are only a few reports of double ray amputation of the hand in the literature.
Case presentation: We treated a 72-year-old man with open fracture of 3rd and 4th finger with vascular compromise. The patient was against amputation and we performed Open Reduction and Internal Fixation (ORIF) surgery at first. Then we perform amputation because the finger saturation didn’t improve. After reviewing the literature, we thought that presentation of our patient’s case might provide useful information for clinicians.
Discussion: In this patient, we found signs of ischemic from the beginning, at the Emergency Room. Oxygen saturation of the 3rd and 4th finger was 0%. However, the decision to amputate is not easy to make. Three days after surgery, there were signs of necrosis so ray amputation was performed to maintain the function.
Conclusions: Ray amputation is usually indicated for tumors, severe trauma, and vascular insufficiency. Compared to proximal phalanx amputation, ray amputation has better cosmesis, better patient satisfaction and similar function.
