Current Updates on Open Fracture Management

Open fracture is arguably one of the most common yet challenging cases encountered by orthopedic surgeons. It has long been recognized as life threatening condition and needs to be managed promptly. It is an injury where the fractured bones and/or hematoma are exposed to external environment, thus increases the risk of infection and further tissue complications. Any fracture associated with a wound in the same region must be considered as open injury until proven otherwise by surgical exploration. The principles of open fracture management are to manage the overall injury and specifically prevent primary contamination to become frank infection. The treatment course itself has been developing over the last few centuries. The very first dialogue regarding exposed fracture treatment dates back to Hippocrates who encouraged occlusive dressings after improvement of the oedema and debridement of the purulent material. Immobilization was widely recognized in 1903, using wood sticks that were tied together by bandages, but it was actually introduced further before, using palm branches. Before antibiotics were developed, severe open injuries often managed by emergency amputations to prevent further infection. Until now, the evolution of open fracture management does not stop. Early identification of an open fracture, proper irrigation and debridement, optimum fracture stabilization and repair, and timely administration of appropriate antibiotics are the primary goals of modern orthopaedic trauma care. This paper discusses the updates on open fracture management, including primary and secondary assesment, choice and timing of antibiotics, debridement and choice of lavage fluids, skeletal stabilization, and wound closure.
