The era of modern thyroid surgery was introduced by Theodore Billroth and subsequently developed as a safe and effective procedure by Theodor Kocher. Since then, excision of the thyroid through a skin crease incision in the anterior neck just over the thyroid gland has been the proven, efficacious way of treating a variety of thyroid pathologies. The open method provides good direct exposure to facilitate safe dissection and a quick operation with low morbidity and minimal mortality. However, these patients still have a scar in the neck. Some scars blend in well with the skin crease while others may heal with hypertrophy. Furthermore, the majority of patients with thyroid disease who require surgery are women who are concerned about the aesthetic appearance of the scar. Rapid and improving technologic innovations have allowed surgeons to perform what would have been previously unthinkable: the removal of the thyroid gland from a remote site, providing a scarless outcome in the neck1.
